REBATE OFFER CONDITIONS

1. Offers available only to adults 21 years of age or older who purchase valid TextRebate products listed on www.app.textrebates.com from stores in applicable states.
2. Only ONE receipt allowed per submission. Receipts must have brand names ITEMIZED. Store name and date/time of purchase must be visible.
3. Receipt submissions must be submitted within 10 days offer expiration date. Receipts submitted more than 10 days after an offer expiration date will not be processed.
4. Offer not valid for employees, spouses or significant others of alcohol beverage wholesalers or retail licensees / employees. TextRebates reserves the right to deny any suspected persons.
5. Refund requests from groups, clubs, and organizations or from officers or employees of alcohol beverage retailers or wholesalers will not be honored or returned.
6. Must allow 4 weeks for delivery of check if rebate is requested by mail. To submit a rebate by mail: send to 7327 SW Barnes Rd. #120, Portland OR 97225
7. TextRebates reserves the right to deny or limit any rebate request.

1. Void where prohibited, taxed or restricted.
2. Reproduction, purchase, sale or trading of this form, UPC symbols or cash register receipts is prohibited, and facsimiles of these will not be honored or returned.
3. Fraudulent submission may result in federal prosecution for mail fraud under 18 USC Sections 1341 and 1342.
